Assessment
The Behavioral Health assessment is a process of gathering information to learn about a person’s needs and ability to function in order to determine the best treatment for them. The assessment includes questions about the presenting problem, an evaluation of their current mental status, and the creation of a diagnostic impression. The outcome is to determine the need for care and recommend appropriate services and/or treatment.
Program Objective
- Diagnostic Assessments are scheduled as the initial appointment to begin services at Pathways.
- Visitors should plan on at least 2 hours for this appointment and should bring their insurance card and list of medications.
- Pathways will ensure that counselors with specialized training in assessments are the primary assessors.

Psychiatric Services
Psychiatric Services provide psychiatric/medical interventions used to reduce and/or eliminate psychiatric symptoms with the goal of improved functioning, including management and reduction of symptoms. Services are available to youth and adults who exhibit severe symptoms of diagnoses best treated with medication, e.g., ADHD, Depression, Bi-Polar, etc., and outpatient therapy. Priority is given to mental health emergencies and referrals from Inpatient Psychiatric Programs.
Our psychiatric services include Medication-Assisted Treatment (MAT) for substance use in the form of Vivitrol® as part of a comprehensive approach to recovery from opioid and alcohol use disorders. Vivitrol is a non-addictive, once-monthly injectable medication that helps reduce cravings and prevent relapse. When combined with counseling, therapy, and ongoing support services, MAT can help individuals build a strong foundation for long-term recovery and improved overall wellness.
Crisis Intervention
Crisis Intervention is the process of responding to behavioral health emergency situations and may include; assessment, immediate stabilization, and the determination of level of care in the least restrictive environment in a manner that is timely, responsive and therapeutic. Pathways maintains a close working relationship with local health care providers, the sheriff’s office, emergency medical workers, and schools.
Crisis assessments are available 24 hours a day, 7 days a week. This service is provided by the licensed clinicians at Pathways from 7:00 a.m. to 7:00 p.m. Monday through Thursday (except holidays) and after hours by Coleman Health Services by calling 1-800-567-4673.
Case Management
Our Case Management Services are designed to help youth and adults navigate the behavioral health system and access the resources needed to achieve greater stability, independence, and overall well-being. As an Ohio-certified behavioral health agency and a CARF-accredited provider, we are committed to delivering person-centered, high-quality services that support each individual’s unique goals and needs.
Our experienced case managers work collaboratively with clients, families, healthcare providers, and community organizations to coordinate care and remove barriers to treatment and recovery. Services may include assistance with accessing mental health and substance use treatment, medical care, housing resources, employment and educational opportunities, financial assistance programs, transportation, and other community supports.
We believe that successful recovery involves more than treatment alone. Through advocacy, care coordination, and ongoing support, our case managers help individuals develop the skills, connections, and resources necessary to live healthier, more fulfilling lives. We focus on empowering clients to make informed decisions, strengthen self-sufficiency, and achieve their personal recovery goals.
Whether an individual is experiencing a new behavioral health challenge or managing long-term needs, our Case Management Services provide guidance, support, and coordination every step of the way.
Gambling Addiction Therapy
Treatment and prevention for problem gambling.
Prevention
Prevention programming is a service that provides youth and adults with education, skill building, leadership opportunities, and alternative activities to encourage positive behavior and to prevent suicide and the use of drugs and the abuse of alcohol. Youth are the main target of prevention programming, but education is available for all age ranges. We provide community awareness campaigns, school-based programs, youth leadership opportunities (see description below), and drug free alternative activities for youth of all ages.
Program Objectives
- Teach youth the necessary skills to resist social (peer) pressure to alcohol, tobacco, and other drugs.
- Increase the utility of identified protective factors within the community.
- Increase/maintain abstinence from substances among youth.
- Increase community knowledge of the consequences of substance/alcohol use, abuse, and addiction.
- Help youth to develop greater self-efficacy.
- Enhance knowledge around crisis intervention and response.
Youth Leadership Opportunities
- P.A.R.T.Y. (Putnam Adolescent Response Team for Youth) – a youth led group whose mission is to use positive peer pressure to encourage positive choices, mental wellness and to prevent underage use of alcohol, tobacco, and other drugs. Meeting weekly, P.A.R.T.Y. is composed of high school students from different schools in the county, and works to empower youth, create alternative activities, educate and create public awareness, and create a network for making positive choices.
- J.E.D.I. (Junior Educational Development Institute) – a youth led conference to help build skills in middle school students. This retreat, which is planned and implemented by P.A.R.T.Y., introduces the students to workshops, motivational speakers, small group process, problem solving, and teambuilding. J.E.D.I.-like programs are also offered to elementary students as “Jumping Into Jedi.”
